COURSE OVERVIEW
Transform Capacity Gains Into Real‑World Performance
Course Description
The Practice Accelerator Forum on Activity Tracking is a 3-hour professional development experience designed to help rehabilitation clinicians translate measurement science into everyday practice. This hybrid format includes a 2-hour on-demand session followed by a 1-hour live mentoring discussion focused on the practical application of activity tracking, goal setting, and motivational interviewing in outpatient rehabilitation.
The session begins with a foundation in measurement theory, using the International Classification of Functioning (ICF) framework to distinguish between capacity and performance and to explore the implications of measurement noise and device variability. Participants will then examine clinical strategies for integrating performance monitoring and related interventions into patient care, including the selection of appropriate tools (e.g., activity trackers, accelerometers, and activity logs), interpretation of data, approaches to documenting and communicating findings, and behavior-change interventions that target performance.
A case example will illustrate how to structure activity-based goals, balance in-clinic treatment sessions with real-world performance, and use data to guide decision-making. Interdisciplinary strategies for embedding activity tracking into treatment planning and for sustaining patient engagement across the continuum of care will be discussed.
The live mentoring session provides an interactive space for clinicians to apply these concepts to their own cases, discuss implementation challenges, and refine performance-focused interventions. Participants will receive a downloadable toolkit with sample logs, patient handouts, and key references to support clinical implementation.
WHAT YOU’LL LEARN
By the end of this course, participants will be able to:
- Differentiate between capacity and performance within the ICF framework and explain how each construct informs clinical assessment and goal setting.
- Identify at least three common sources of measurement noise or variability when using activity‑tracking tools (e.g., accelerometers, step counters, activity logs).
- Select appropriate performance‑monitoring tools and interpret the resulting data for clinical documentation and interdisciplinary communication.
- Develop measurable, performance‑focused goals that connect in‑clinic therapeutic activities to improvements in real‑world occupational performance.
- Apply one evidence‑based behavior‑change strategy to support improved daily activity performance.
- Design an individualized intervention plan that integrates performance data to support meaningful functional outcomes across home and community contexts.
- Analyze a case example and justify clinical decision making based on activity‑performance trends, environmental factors, and patient‑identified priorities.
